How lip filler works in the lip

Hyaluronic acid is a sugar molecule your own skin already makes. It holds water, and that is the whole mechanism: a gel of cross linked hyaluronic acid placed in the lip draws water to itself and occupies space, so the tissue sits fuller and the border sits more clearly than it did. Nothing is cut and nothing is implanted permanently. The body breaks the gel down over months with its own enzymes until the lip returns to how it started.

Lips need a soft, loosely cross linked gel. A firm gel resists being squashed, which is exactly what you want deep on bone for a chin or a jawline, and exactly what you do not want in a structure that moves every time you speak, eat and smile. Choosing the wrong firmness is one of the commonest reasons lips look and feel overdone rather than fuller.

The four things assessed before anything is injected

Lip treatment is a proportion problem before it is a volume problem. Four things decide the plan.

• The ratio of upper to lower lip. A balanced mouth usually carries more volume in the lower lip than the upper. Filling both equally is what flattens a mouth and makes it read as treated.
• The vermilion border and the cupid’s bow. Definition here is often what people are actually asking for when they ask for fuller lips, and it takes very little product placed accurately.
• Lip height and the distance to the nose. A long upper lip with a thin red portion behaves differently from a short one, and adding volume to the first can make it look heavier rather than fuller.
• Support from behind. Lips sit on teeth and in front of a chin. A flat side profile is sometimes a chin question rather than a lip question, and chin and jawline filler is the treatment that answers it.

Lip filler is a shaped gel. It has a position and a shape, and it is the right treatment when the lips need volume, projection or a defined border they do not currently have.

Skinfill Bacio is a hydrating lip treatment, also from £250. It improves the quality, hydration and surface of the lip rather than building a shape, so it suits lips that are dry, lined or dehydrated but not short of volume, and people who do not want their lip shape changed at all.

Some plans use one, some the other, and some use both for different jobs. The choice is made at the assessment, not from a price list.

How long lip filler lasts, and what shortens it

Lip filler typically lasts six to twelve months. Lips are the shortest lived filler result on the face, because the area moves constantly and movement breaks the gel down faster. For comparison, cheeks and midface usually hold twelve to eighteen months and structural work on a chin or jawline can hold up to two years.

Those are ranges rather than promises. A fast metabolism, regular high intensity exercise, heavy sun exposure and smoking all shorten the life of a result. A review at two weeks and a planned top up before the result has fully gone keeps the shape consistent and usually needs less product than starting again.

The first two weeks: swelling, bruising and settling

You will see a change as you leave, because the gel occupies space immediately. What you see on day one is not the result. Lips swell more than any other area of the face: swelling peaks in the first twenty four to forty eight hours, settles over the following week and has usually resolved by two weeks, which is when the outcome is assessed and when photographs are taken.

Bruising is possible anywhere a needle enters skin and is more likely in lips than on cheeks or a jawline. It typically fades within a week and covers easily. Small firm areas that soften, tenderness and a slightly uneven feel in the first days are all normal. Avoid strenuous exercise, alcohol, saunas and steam rooms for twenty four hours, and avoid massage or facials over the area for two weeks. If you are prone to cold sores, tell us at consultation, because treatment can bring an outbreak on and that is managed in advance.

Plan around your diary rather than the other way round: two weeks before an event, not two days.

Risks, side effects and how they are reduced

Every injectable treatment carries risk, and a clinic that does not discuss it is not giving you what you need to consent properly.

Common and self limiting: swelling, bruising, redness at injection points, tenderness and short lived asymmetry while swelling settles.

Uncommon: a firm area or nodule that needs massage or, occasionally, dissolving; a result that is not what you wanted, which with hyaluronic acid can be adjusted or reversed; a cold sore outbreak in someone who carries the virus; delayed inflammatory swelling, which can appear weeks or months later, often after an infection or a vaccination, and is managed medically.

Rare and serious: vascular occlusion, where product interrupts the blood supply. It is reduced by anatomical knowledge, slow injection, aspiration where appropriate and small volumes, and it is treated urgently if it happens. You are given the clinic’s contact details and told exactly what to look for and when to call before you leave.

Dissolving and adjusting lip filler

Because only hyaluronic acid fillers are used here, lip filler can be broken down with an enzyme called hyaluronidase, usually within a day or two. That reversibility is the main safety advantage of hyaluronic acid over permanent and semi permanent products, which is why those are not offered.

Dissolving is assessed rather than sold. It is appropriate for a firm nodule, for product in the wrong place, for migration above the border, or for a shape you no longer want, and it is assessed at a consultation first, including where the original treatment was carried out elsewhere. Hyaluronidase itself carries a risk of allergic reaction and removes your own hyaluronic acid alongside the gel, so it is not a routine step and the lips are allowed to settle for several weeks before anything is replaced.

Treatment is declined, or deferred, if you are pregnant or breastfeeding; if you have an active cold sore, skin infection or inflammatory condition at or near the lips; if you have had a recent vaccination or are unwell with an infection; if you have a known allergy to hyaluronic acid or lidocaine; if you have an autoimmune or connective tissue condition your specialist has advised against injectables with; or if you are taking a medication that makes the treatment unsafe.
